The Cost of Misdiagnosing Postpartum Psychosis

The tragic consequences of misdiagnosing postpartum psychosis are becoming impossible to ignore. Across the country, new mothers who desperately try to communicate their symptoms are often dismissed as simply anxious or overtired. This failure to distinguish between common postpartum mood changes and a true psychiatric emergency can have devastating outcomes.

Postpartum psychosis is rare, affecting roughly one to two women per thousand births, but it is a medical emergency. Unlike the "baby blues" or even postpartum depression, this condition involves a break from reality. Symptoms can include rapid mood swings, confusion, paranoia, hallucinations, and delusions often centered on the baby. A mother might believe her child is possessed or that she must harm the infant to save it. These are not exaggerations of normal stress. They are signs of a brain in crisis.

The problem is that many healthcare providers, and even family members, mistake these extreme symptoms for severe anxiety or exhaustion. A woman who says she hears voices or believes her thoughts are being controlled might be told to get more sleep. This delay in proper psychiatric care can be fatal. Studies show that women with untreated postpartum psychosis are at significant risk for suicide and infanticide.

The solution requires a shift in how we listen. When a new mother says something is fundamentally wrong, we must assume she is telling the truth until proven otherwise. Emergency rooms and obstetric units need better protocols to screen for psychosis, not just depression. Family members need to know that bizarre behavior, especially involving fixed false beliefs, is not a cry for help but a call for immediate intervention. The cost of misdiagnosis is measured in lives lost. We cannot afford to get it wrong.
